Patients referred to Isaac Health are seen virtually by a cognitive specialist within days, not months. We provide comprehensive evaluation, treatment, and ongoing care management for patients and their families, while ensuring referring providers remain fully informed throughout the process.

Rooted in the nation's top dementia care programs

Our program is built on the foundations of evidence-based, nationally recognized dementia care programs, including UCSF Care Ecosystem, UCLA ADC program, and Johns Hopkins MIND At Home Program.

When to loop us in

• New or emerging cognitive concerns: If your patient is experiencing new issues with memory, language, thinking, or attention, our specialists can provide a timely and accurate evaluation.
• Worsening or progressing symptoms: For patients with diagnosed or suspected dementia whose symptoms are intensifying or affecting daily function, we offer comprehensive assessments and personalized care plans to improve quality of life.
• Complex symptoms: If cognitive changes in your patient are accompanied by behavioral or neuropsychiatric symptoms (such as apathy, agitation, depression, or anxiety), our interdisciplinary team can support differential diagnosis and targeted management.

Multidisciplinary dementia care delivered through telehealth

Specialist Assessment

Comprehensive neuropsychological and neurological assessments to ensure an accurate diagnosis. Includes neuropsychological testing and lab imaging as needed.

Treatment

Our specialists work with each patient and caregiver to create a personalized treatment plan, which may include cognitive rehabilitation therapy, medication treatments, and new disease-modifying therapies where clinically appropriate.

Ongoing care management

We support families throughout the care journey with care navigation, skills training, and caregiver education. Our approach aligns with the CMS GUIDE Model and emphasizes person-centered, evidence-based care.
